STUDENT PHYSICAL FOR SPORTS OR WORKING PAPERS

  • Any student interested in playing sports is required to have a valid physical on file in the school health office. All medical paperwork, such as sport physical forms, health interval forms, and any document from the child's medical provider must be returned to the Health Office. If the Health Office is closed then contact your child's coach or the Athletic Office. A physical may be given by the child's private physician or the school doctor. The physical is valid if it has been completed within 12 months of the start of each sport season.

    Please note: a work-up is required before a physical may be given.

    A physical is also required as part of the application process for working papers. Students wishing to obtain working papers should plan on getting their work-up and physical at school, or bring in a valid physical from their own physician. Approved physical forms are available by clicking on the link or upon request from the health office.
