Health Screenings
A required New York State School Health Examination Form must be completed in entirety by a private health care provider or school medical director. Note: New York State Education Department requires a physical exam for new entrants and students in grades Pre-K or K, 1st, 3rd, 5th, 7th, 9th and 11th; annually for interscholastic sports; and working papers as needed; or as required by the Committee on Special Education (CSE) or Committee on Pre-School Special education (CPSE).
Health screenings will be conducted as needed throughout the school year. Please note the grade levels where each screening will take place. Parents/guardians will be notified in writing of any abnormal findings.
- Vision screening is required for grades Pre-K or K, 1st, 3rd, 5th, 7th, 9th and 11th.
- Hearing screening is required for grades Pre-K or K, 1st, 3rd, 5th, 7th, 9th and 11th.
- Scoliosis screening is required for grades 5th and 7th for girls and grade 9th for boys.
